Rights & Responsibilities of Students with Disabilities

Regarding Access to Minnesota State University Moorhead

• I understand that I am registering for services from Accessibility Resources (AR) at Minnesota State University Moorhead and that I may be eligible for services such as information, referral, accommodations and/or other individualized services that may be needed for access to courses, activities, programs, services, and/or facilities. If Accessibility Resources is unable to provide the necessary services, I may be referred to other appropriate University Departments or community agencies.
• I understand that as a user of Accessibility Resources, I am responsible for reviewing the rights and responsibilities pertaining to disability access (see below).
• I understand that I will not be eligible for services if I do not provide documentation of a diagnosed disability, do not have a diagnosed disability, or do not follow Accessibility Resources and University policies and procedures.
• I understand that if I request Accessibility Resources to facilitate accommodations on my behalf, they may need to consult with other University personnel. I give my permission to have disability-related information shared with appropriate University personnel to facilitate such requests.

It is my right to have:

• Appropriate confidentiality of all information regarding my disability and to choose to whom, outside of the University, information about my disability will be disclosed, except as disclosures are required/permitted by law
• Information reasonably available in accessible formats

It is my responsibility to:

• Meet qualifications and maintain essential institutional standards for courses, services, and activities
• Self-identify as an individual with a disability, when an accommodation is needed, and to seek information, counsel, and assistance as necessary
• Document (from an appropriate professional) how the disability limits my participation in courses, programs, services, or activities
• Follow published procedures for obtaining information, services, and accommodations
• Meet with the Director of Accessibility Resources at least once during each semester to discuss progress
• Inform Accessibility Resources of any change in school status (e.g., address change, classes added or dropped, change in disability status, exit from the university)
• Report any problem in the receipt of services to the Director
• Provide each instructor with my Accommodation Letter within one week of receiving my Accommodation Letters
