Authorization to Credit Account Used to authorize MSUM to credit any remaining funds to miscellaneous charges billed to your account, including MSUM Bookstore charges, library fines, parking tickets, late fees, etc.

Used to identify courses and degree program if you are pursuing a second bachelor's degree, certification, licensure, or undergoing preparatory work for a graduate program at MSUM
Dependency Support Verification
