Here is a list of common MSUM financial aid forms and a brief description of their purposes.

| Authorization to Credit Account | Used to authorize MSUM to credit any remaining funds to miscellaneous charges billed to your account, including MSUM Bookstore charges, library fines, parking tickets, late fees, etc. |

Used to identify courses and degree program if you are pursuing a second bachelor's degree, certification, licensure, or undergoing preparatory work for a graduate program at MSUM |
| Dependency Support Verification 2024-2025 2025-2026 |
Used for to document that you provide over half the support of a dependent child while you are enrolled. |
