Description
This electronic form may be completed by any member of the university community, alleging a student or student organization has violated student conduct code policies. Reports should be submitted as soon as possible after the event takes place. Upon submission of this report, the director of Student Conduct and Resolution shall conduct an investigation of the allegations. The completed, printed report should be submitted to the Office of Student Conduct and Resolution in Flora Frick 153. |

Description
This printable form is used for students who are required to complete community service hours as assigned by a student conduct sanction. The completed, printed form, with verification signature at the bottom, should be submitted to the Office of Student Conduct and Resolution in Flora Frick 153, or to the hearing officer the student met with. |
