    The Department of Homeland Security (DHS) requires all international students in F-1 and J-1 status to report address changes within 10 days of any move. This also includes period of Optional Practical Training (OPT), Academic Training (J-1) and change of address, employment and unemployment periods. During OPT period specifically, students are also required by law to report employer information each time they accept a position, as well as any period of unemployment. During a student's OPT STEM EXTENSION period, students by law are required to report employer and employer contact information 6 and 12 months even if the information is the same since the last reporting. If you do not report both 6 and 12 months reporting, SEVIS system is automatically set to terminate your record. Please use this form to make necessary changes. Once you complete this form, it will automatically be sent to International Student Services, who will report to DHS electronically.
